Blind playtester report form -- anyone got one?

I'm starting the blind playtest phase on a boardgame, and I need a form to send out to the playtesters for them to record their thoughts, opinions and verdicts. I was wondering if anyone had a standard form I could look at, to get an idea of 'prior art'.

I'm kind of startled that there isn't one already in the Downloads section. Obviously each game design is different but there'a standard data about each one that could be put together on a generic, customisable form that would save a lot of people a lot of trouble.

If I'm satisfied with the one I'm working on, I'll offer it up for download.

The yahoo protospiel group http://games.groups.yahoo.com/group/protospiel/ has a few different ones in PDF and DOC format that have been used at past events.

What To Look For In Playtesting steve jackson games

Do the rules work?
Do they pass the "reality check?"
Is the text easy to read and understand?
Is it FUN?
Is it what you expected? (better? worse?)
Does the author exclude any elements you wanted to see?
Does he include anything you found irrelevant or unnecessary?
If the draft seems long, what should we cut or trim?
What should we absolutely not take out?
If you see rules problems, how would you solve them?
Do these rules take the system in any directions that we may regret later?

Many thanks for the link to the Steve Jackson site. I have a meeting with Steve next week; I'll ask if he's got any forms created for internal SJG use that might be more complete.
