Is it necessary to have a game published? I want to create a game for dementia patients and offer it for sale on my web site. When I'm finished I'd like to get it copywrighted. Is this ok? And, finally, if I don't go through a publishing house, what steps do I take to get my idea manufactured? Thanks for your time in answering my questions.

Publishing simply means that
Publishing simply means that the game is being made available in one form or another. If you produce and make it available for sale on your website you are the publisher.
As the publisher you’ll not only need to make sure that the game works well you will need to choose and price components, art, packaging, rules layout & printing. That’s just to end up with a game to sell. At the same time you’ll need to develop a good sales plan and manage logistics like warehousing, compliance with state/federal law and bookkeeping.
In other words you’re starting a business, which requires planning. If you do want to go down that road and don’t have a business degree get yourself a good book on the subject.
I would start at the beginning get the game through rigorous testing before deciding if its going to be worth it to self publish.
