Is anyone familiar with a squad level cooperative modern warfare game? Think something like Hero Quest but with M-16s, RPGs, jungles, post-apocalyptic cityscape, or whatever.

Squad level cooperative modern warfare game?
If you're allowing "post apocalyptic", then Mutant Chronicles is sort of the same (though a lot more funky than M-16s). It's co-op in the sense that a team of players cooperate against a single other player who manages the mutants, not pure co-op in the sense of everyone against the game.
