You definitely want to include the Name, the Number of Players, the Age Range, and an indication of what type of game it is. This last doesn't necessarily have to be in text. For example, few people would mistake Candyland for being an abstract strategy game.
If you're planning on getting the game on retailer shelves, it's also a great idea to include a description of how the game plays. This is both so that customers can get a good idea of whether or not they want to play, and retailers can effectively sell the game. You also want to have the UPC on the box. Seems silly to say, but we forgot that with Ice Lake, and has to use stickers. Nothing quite as mind-numbing as sticking UPC barcode stickers on game after game after game after game...
I've always been partial to the little information boxes that indicate how much strategy is involved vs. how much luck.
I like it when a game have some "luck", "strategy", "social" kind of rating on the side of the game just to describe what type of game it is.
I have to agree that I like to have the components list and a photo of game components. Depending on the game type, you can also include information on one of the sides. For my tuck boxes, I used one side for information that wouldn't fit on the back and I used the bottom for the bar codes. You probably won't have a problem fitting everything on a board game box, but it's something to think about.
I'd also recommend Spielboy's invaluable guide to those strange icons you find on the side of professionally published games.
Have a look here.

(edit: and yes, I know that's not a serious link.)

The horror themed icons on Finstere Flure are a particularly nice touch, I always think.
